Instructions:
1. Locate the form you wish to complete in the list below.
2. Choose the form and file format of the document you wish to download by clicking on the appropriate file name below. Note that both Microsoft Word (doc) and Adobe Reader (pdf) formats are available.
3. If the form does not automatically open on your computer, locate the downloaded form on your local computer (it is often on your desktop or in your Documents folder) and double-click the form to open it.
4. If you are using the Alternate Media Request Word form, please print and sign your form before submitting it to the DRC office. The Alternate Media Request PDF form can be submitted by clicking the "click here to submit" button on the top right corner of the form.
5. After completing the selected form, use the table information at the bottom of the page to determine appropriate mailing, faxing, or emailing instructions for your form.
