You will receive a copy of your ASE form in your club mailbox once the process has been completed. Have your copy of the form with you when you are at your meetings.

FOOD EVENT PROCEDURES
To set up a food event for your club or organization, you will need to do the following:

Your Club/Organization must be currently chartered (on-line as well as your Advisor Form and Executive Officer Signature Form turned in)

Have a certified food handler available (usually a club member)

Plan ahead! Advance planning is very important.

Have a few dates and locations in mind for your event.

Check with Alia regarding location/time and paperwork.

Have all documents signed by club officer, club advisor, and Environmental Health and Safety.

Return the Application for Scheduled Event form to Cecilia Lopez and keep the Environmental Health and Safety form to have in your possession for your event!
