How to Make an Appointment

To schedule an appointment to see a doctor, nurse practitioner or health educator (check hours of operation), please call our main phone number, (909) 869-4000, and then press "0" to speak to a receptionist. WE REGRET THAT WE ARE UNABLE TO PROCESS YOUR REQUEST VIA E-MAIL AT THIS TIME.

If you have a preference for a specific clinician, let us know so that we can schedule you with your choice. Be prepared to give the following information when asked:

• A brief description of the length and duration of your symptoms
• Your current status as a Cal Poly Pomona student
• Whether or not you are a new patient

• Your Bronco ID number 

As a patient, you should also be aware of your rights and responsibilities. Also know that your medical records are confidential and that Student Health Services must adhere to strict legal guidelines to ensure your privacy. 

New Patients 

As a new patient, you will need to allow extra time before your appointment to fill out some medical history forms. Plan to arrive about 20 minutes before your scheduled time and check in with the receptionist at the front desk. Remember to sign the parking log, if you parked in our lot, and be sure to bring your Bronco ID to determine your eligibility.

IF YOU ARE UNDER 18 YEARS OF AGE, you will be required to bring in a completed Minor Consent form before being seen.

You will be asked to fill out a medical history report. Make sure you fill out the paperwork completely, especially the reason for the visit, so that our staff might better serve you. Once the front desk has received your paperwork, they will direct you to the patient exam station where your appointment is scheduled.

Proceed to the appropriate treatment area as directed by the receptionist, and wait for your name to be called.

As a good precautionary measure, if you are covered by an outside insurance company, plan to carry your insurance card or a copy of your parent's card, so that necessary medical services can be provided in a timely fashion should the need arise. 

Continuing Student

If you are a Continuing Student (have attended classes at least 2 consecutive quarters previously, are not currently enrolled, and intend to resume classes the following quarter), you will be charged $15 for each visit up to a total of $45/quarter. Payment may be made with cash, check, Mastercard or Visa at the time services are rendered.

Cancelling Your Appointment

If you need to cancel your appointment, please call 909-869-4000 and speak to a receptionist during our business hours. We appreciate your notifying us at least 24 hours in advance so that we might schedule another patient in your time slot.
