Personalized Assessments

A variety of health assessments and screenings are available to you free of charge through the SHS Wellness Center. Some assessments may require an appoinment. However, the majority of the assessments can be completed on your own time schedule. Hard copies of the written assessments may be printed from the list below or picked up at our Health Resource Room. Once completed, return the paper assessment(s) to the SHS Wellness Center for personalized feedback.

For a private appointment, call SHS Wellness Center at (909) 869-5272 to schedule an appointment with a health educator. Or walk in to the Wellness Center during the hours of operation, and speak to a Peer Health Educator.

Caffeine Use Quiz (1. Take the "Caffeine Addict" quiz - "Enter Quiz" at top; 2. Click on the caffeine calculator to determine your daily caffeine intake; 3. Use this result to take the Risk test, and print the results as proof of completion)

Hearing Assessment (Print results to show proof of completion)
Oral Health Assessment (Print the results page to show proof of completion)
Sexual Health Screening (take 5 minutes to see if you need to get tested for common STIs or Sexually Transmitted Infections. Print final page for proof of completion)
